Position Summary
The Case Manager at West Ascension Parish Hospital is responsible for coordinating comprehensive, patient-centered care across the continuum—from admission to discharge—ensuring optimal resource utilization and high-quality outcomes. This role requires strong clinical judgment, knowledge of community resources, and collaborative communication with medical providers, nursing staff, social services, and external agencies.
Working in a rural critical access hospital, the Case Manager supports transitions of care, facilitates discharge planning, monitors length of stay, and serves as an advocate for patients and their families. This position reports directly to the Chief Nursing Officer (CNO) and collaborates with multiple departments to support compliance with state and federal regulations.
Essential Duties and Responsibilities
· Conduct initial and ongoing case assessments for all inpatients and applicable outpatients.
· Develop individualized discharge plans in collaboration with physicians, nursing staff, patients, and families.
· Coordinate post-discharge services, including home health, DME, hospice, rehabilitation, and skilled nursing placements.
· Monitor length of stay and assist with concurrent review to ensure appropriate resource utilization.
· Facilitate communication between patients, providers, payers, and support services to reduce barriers to care.
· Identify high-risk patients and intervene early to support care continuity and prevent readmissions.
· Collaborate with billing and utilization review staff to support reimbursement processes and documentation.
· Ensure compliance with Medicare Conditions of Participation and Joint Commission standards for discharge planning and case management.
· Maintain detailed case documentation and reporting using hospital EMR and care coordination systems.
· Serve as a liaison between the hospital and community agencies, including social services, elder care, and behavioral health providers.
· Participate in quality improvement initiatives and interdisciplinary care team meetings.
Qualifications, Education & Experience:
Associate’s or Bachelor’s Degree in Nursing (ADN/BSN) required.
Current RN licensure in the State of Louisiana required.
Minimum of 3 years of clinical experience in acute care, with at least 1 year in case management, discharge planning, or utilization review preferred.
Experience in rural or critical access hospital settings preferred.
Knowledge, Skills & Abilities:
· In-depth understanding of hospital discharge planning, medical necessity criteria, and continuum of care services.
· Knowledge of Medicare/Medicaid guidelines, commercial insurance processes, and community healthcare resources.
· Strong communication, assessment, and organizational skills.
· Proficiency in EMR systems and basic Microsoft Office applications.
· Ability to work independently and collaboratively in a fast-paced clinical environment.
Working Conditions:
Primarily hospital-based, with regular patient and interdisciplinary team interaction.
Monday–Friday schedule with occasional weekend or on-call responsibilities, as needed.
