THE POSITION: The Community Health Worker (CHW) is responsible for helping LGBTQIA Staten Islanders and their families to navigate and access healthcare services, community services, and adopt healthy behaviors. The Social Care Navigator/Community Healthcare Worker (CHW) is responsible for outreaching and engaging with Medicaid members telephonically and in person to assess health-related social needs (HRSN) and navigate members to health and social care services. The Social Care Navigator/CHW will use designated technology platforms and mobile devices to conduct outreach and screening in various community-based settings. The candidate may be assigned to various settings including physician practices, clinics, and/or community centers on a rotating basis. The Social Care Navigator/CHW builds trust with community members and assists them with accessing care at all levels of the continuum, and coordinating referrals to community services, programs, and Health Homes, as needed. The role involves approximately 80% field work on Staten Island with approximately 20% office time for followup and team meetings.
This is a Full-time position (40 hrs/week). The CHW will be expected to begin, be working towards, or have completed the Community Health Worker Certificate Training Program.
COMPENSATION RANGE: The compensation range associated with this position is $55,000 a year.
PRIMARY RESPONSIBILITES:
- Responsible for establishing trusting relationships with clients and, when appropriate, their families while providing general support and encouragement
- Make referrals to LGBTQIA affirming health care and community services
- Manage a caseload of assigned clients and conduct outreach, HRSN screening, and comprehensive navigation for referrals to social care services.
- Use technology platforms to document client eligibility, outreach activities and case notes, outcomes of referrals, and other tasks as required.
- Collaborate and communicate with team members, partner-based navigators/CHWs, and community partners to manage members with complex needs.
- Provides members with accurate information concerning health care benefits and coverage.
- Report to supervisor with outreach and navigation updates. Attend weekly care team meetings and other meetings to review outcomes and performance.
- Performs other duties as assigned.
- Attend trainings as well as supervisory and all-staff meetings as assigned
- Maintain HIPPA compliance at all times

QUALIFICATIONS:
- A strong commitment to the Pride Center of Staten Island’s mission, values, and goals.
- Community Health Worker Certificate Required
- High School Diploma, High School Equivalency (HSE), or General Educational Development (GED) required
- Ability to lift 30 lbs.
- Bi-lingual (Spanish/English) preferred but not required.
- Three (3) years community health or navigation experience preferred.
DETAILS:
- Location: Pride Center of Staten Island, 66 Willow Ave, Suite 202.
- Work Schedule: Tuesdays and Wednesdays 10-6PM; Thursdays, 12 PM – 8 PM; Fridays and Saturdays, 10 AM – 6 PM. Work schedule and location are subject to change in relation to event preparation and execution.
