Position Overview :
Pride Health is seeking a skilled RN Case Manager to join the team. This contract position is ideal for a dedicated professional with experience in case management and clinical care. The role involves managing high-risk patients, providing a seamless continuum of care, and working with a multidisciplinary team to improve patient outcomes while ensuring resource optimization.
Key Responsibilities :
- Supervise and support high-risk patients.
- Design and manage a continuum of care for clients.
- Perform assessments, service planning, and resource acquisition.
- Link clients with community service providers.
- Ensure effective use of healthcare resources through utilization review.
- Work with a multidisciplinary team to reduce care fragmentation and improve patient outcomes.
