Pride Health (TJC Certified) is looking RN Case Manager to support one of our largest healthcare clients in Greenbrae, CA 94904.
Hospital discharge planning exp REQUIRED!
Contract Duration: 13 weeks
Shift: Afternoons; 40 hours/week, Day shift (0800-1630) with rotating weekends.
Pay Range: $2900 - $3000
Requirements:
- Must have recent strong acute care hospital discharge planning experience.
- Conducting patient initial assessments
- Acute Care hospital experience
- EPIC EMR documentation experience
- Active CA RN License
