Job Title : LCSW (Licensed Clinical Social Worker)
Location : Paramus, NJ 07652
Shift : Day 5x8-Hour (08 : 00 - 16 : 30) 40 Hours weekly
Duration : 13 Weeks with the possibility of extension.
Pay Range : $2880 / Week.
Job Summary :
Screen, Assess, and Evaluate patients and their families to identify psychosocial needs and develop appropriate interventions. Confirm discharge plan for patients. To educate, counsel, and refer as indicated. To plan and communicate effectively with multi-disciplinary team and community resources.
Education :
Master's Degree in Social Work
License Requirement :
NJ State License - LCSW
Skills : Hospital experience
Discharge planning experience
