Pride Health is seeking a dedicated and experienced Social Worker to join our team in Greenville, South Carolina. This contract position is ideal for a professional with a strong background in social work and patient care. The role involves providing comprehensive social services to patients, conducting assessments, and creating care plans, while ensuring adherence to legal and regulatory standards.
Key Responsibilities:
- Patient Assessments: Conduct thorough evaluations of patients' social, emotional, and financial needs.
- Care Planning: Develop and implement care plans tailored to individual patient needs, collaborating with multidisciplinary teams.
- Intervention: Provide direct services to patients, including counseling, advocacy, and linkage to appropriate resources.
- Documentation: Maintain accurate and timely records of assessments, interventions, and patient progress in compliance with legal and regulatory standards.
- Collaboration: Work closely with medical teams, other social workers, and outside organizations to ensure comprehensive patient care.
Qualifications:
- Licensure: Must have a valid South Carolina (SC) LCSW license.
- Experience: Minimum of 2 years of experience in social work required.
- Experience with EPIC EMR required.
- Must be willing to float to other units as needed.
- Flu vaccine required (no exemptions).
- COVID-19 vaccine is not required.
Salary : $50 - $55
