Pride Health (TJC Certified) is looking Nurse Practitioner / Physician Assistant to support one of our largest healthcare clients in Reading, PA Berks County .

Title : Advanced Practice Clinician- Nurse Practitioner / Physician Assistant
Location : Reading, PA Berks County
Duration : Approximately 4-7 months
Schedule : 40hrs. / week
Shift : Monday through Friday; 8am - 5pm
Pay Range : $72 / hr. - $75 / hr.
Job Responsibilities : Conduct comprehensive assessments. Effectively manage acute and chronic medical and behavioral conditions. Ensure accurate and complete documentation, including ICD 10 conditions. Communicate and collaborate with the interdisciplinary care team. Conduct advanced illness and advanced care planning conversations. Provide patients and caregivers with counseling and education.
Skills & Qualifications :
Recent clinical experience in practice (long-term care / SNF setting preferred)
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Minimum Education Requirement : Master's
