Annual Salary Range : $62,000.00 - $85,000.00
Bonus - $15,000 sign-on bonus for candidates who commit to a 2-year term.
Job Summary :
We are seeking an experienced and compassionate Social Worker to join our team. The successful candidate will work in the Emergency Department, helping to coordinate care and support patients' psychosocial needs. The role is part of a collaborative Care Management Triad Team, working alongside RN Case Managers and Utilization Management professionals.
Key Responsibilities :
Provide psychosocial support and coordination for patients in the Emergency Department.
Resolve barriers to care, including social, emotional, and financial issues, to support timely discharge.
Collaborate with the Care Management team (RN Case Managers and Utilization Management) to ensure appropriate patient care and progression.
Assist with discharge planning, ensuring that patients’ social needs are addressed.
Qualifications :
- Required : Master’s degree in Social Work (MSW) or Master of Social Services (MSS) from an accredited school.
- Required : LCSW or LMSW licensure prior to start date.
- Preferred : Experience in Acute Care Social Work or Emergency Department settings.
- Preferred : CCM (Certified Case Manager) certification.
- Special Requirement (for Maternal-Child positions) : Experience working with neonates, children, and families.
