What are the responsibilities and job description for the Social Worker position at Pride Health?
Pride Health is hiring a Social Worker to support our client’s medical facility in Fort Wayne, IN. This is an 8-week contract with the potential for an extension and a great way to start working with a top-tier healthcare organization.
Key Responsibilities:
(A)Lead Group Therapy Sessions:
- Facilitate group therapy sessions for IOP adult clients three days per week, with each session lasting three hours.
- Foster a safe, supportive, and non-judgmental environment for clients to process their challenges, share experiences, and develop coping strategies.
- Employ evidence-based therapeutic modalities (e.g., Cognitive Behavioral Therapy, Dialectical Behavior Therapy, etc.) to address mental health and/or substance use issues.
(B)Treatment Planning:
- Conduct comprehensive social histories to understand the background and needs of clients.
- Develop individualized treatment plans in collaboration with clients, ensuring they align with clinical goals and recovery needs.
(C)Insurance Reviews:
- Complete insurance reviews to assess the coverage and benefits for IOP clients.
- Collaborate with insurance providers and administrative staff to ensure continued service authorization and address any insurance-related concerns or issues.
(D)Assessments:
Conduct Level of Care Assessments for adolescents or adults in crisis to determine appropriate treatment levels and make care recommendations.
(E)Collaboration and Documentation:
Maintain accurate and timely clinical documentation, including progress notes, treatment plans, and session summaries, by agency policies and licensing requirements.
Required
- Must have an MSW or MSS (no exceptions), as well as must be LCSW or LMSW
- A master’s degree in psychology, social work, or another counseling-related field.
- Licensed MSW as required by the state.
- CPI required within 60 days of employment.
- BLS required within 30 days of employment.
- 2 years of previous experience in the health care or social service field required, as well as psychiatric hospital experience.

Salary Range: $56,118 - $84,177
