Pride Health is hiring a LICENSED CLINICAL SOCIAL WORKER ( LICSW )to support our client’s medical facility based in TOLEDO, OH. This opportunity is a great way to start working with a top-tier healthcare organization!
RESPONSBILITIES :
- Conduct comprehensive assessments to develop individualized care plans
- Facilitate individual counseling and crisis intervention
- Assist patients in navigating protective services (Child Protective Services, foster care, guardianship, Power of Attorney, etc.)
- Collaborate with internal and external healthcare teams for care coordination
- Educate patients and caregivers on discharge plans and treatment options
- Work with patients experiencing psychiatric disorders, substance use, and self-harm risks
- Adhere to NASW Code of Ethics and best practices for home healthcare
