What are the responsibilities and job description for the Social Worker position at Pride Health?

Pride Health is hiring a LICENSED CLINICAL SOCIAL WORKER ( LICSW )to support our client’s medical facility based in TOLEDO, OH. This opportunity is a great way to start working with a top-tier healthcare organization!

RESPONSBILITIES :

Conduct comprehensive assessments to develop individualized care plans
Facilitate individual counseling and crisis intervention
Assist patients in navigating protective services (Child Protective Services, foster care, guardianship, Power of Attorney, etc.)
Collaborate with internal and external healthcare teams for care coordination
Educate patients and caregivers on discharge plans and treatment options
Work with patients experiencing psychiatric disorders, substance use, and self-harm risks
Adhere to NASW Code of Ethics and best practices for home healthcare

UNIT INFORMATION :

Average Daily Census : 4-5 patients

Patient Age Group : Pediatrics, Adolescents

Ratio : 1 : 5

Work Settings : Case management, outpatient clinic, physician offices / private practices

REQUIRED QUALIFICATIONS :

Licensure : LISW or LPCC (Must be active)

Experience : Minimum 2 years of experience in case management or outpatient mental health settings

Offered pay rate will be based on education, experience, and healthcare credentials.
