This opportunity presents an amazing assignment with the added possibility of extension or securing a permanent position with a premier healthcare provider.Key details : Shift : 8 Hour shift, 8a- 5p, (M-F)Guaranteed Hours : 40 Hours a WeekWeekly Pay Range : $2200.00 – $2320.00Qualifications : Must have a social worker license to work in a clinical setting.Must have BLS from AHA.Must have a case management experience.Responsibilities : Assess patients' psychosocial needs and develop individualized care plans.Coordinate patient discharge planning, including arranging follow-up care and services.Provide emotional support and counselling to patients and their families.Collaborate with healthcare teams to address social, financial, and emotional barriers to care.Connect patients with community resources and support programs.Ensure proper documentation of patient interactions and care plans.
