Pride Health is hiring a Social Worker to support our client’s medical facility, which is based in Oklahoma City, Oklahoma. This permanent opportunity is a great way to start working with a top-tier healthcare organization!
Responsibilities:
- Patient Support & Counseling: Provides social services, conducts psychosocial assessments, and counsels patients and families on medical, emotional, and financial concerns.
- Crisis Intervention & Risk Assessment: Identifies high-risk cases, provides crisis counseling for issues like abuse and neglect, and documents these sessions.
- Discharge Planning & Referrals: Assists with discharge planning, evaluates patient needs, and coordinates referrals to community or post-acute care facilities.
- Advocacy & Ethical Support: Acts as a patient advocate, supporting informed decision-making, end-of-life care, and ethical dilemmas.
- Administrative & Supervisory Duties: Maintains records, evaluates service effectiveness, prepares reports, and may supervise students or new employees.
Qualifications:
- Master’s Degree in Social Work from an accredited school.
- LMSW
- Hospital experience

Pay Range: $44.00 per hour (Negotiable)
*Offered pay rate will be based on education, experience, and healthcare credentials.
