PRIDE Health is seeking a LPN / LVN Long Term Care LTAC - Long Term Acute Care for a travel job in Malone, New York.

Shift : 8H Night \n\nAssignment for 16-26 weeks \n\nLPN for LTAC\n\nEvery other weekend, no call, no self-scheduling but may pick up shifts.\n\nRequirements : AHA BLS, please attach any others LTC experience A minimum of 1 year of experience LPN is required to cover CNA duties as needed NY license, or proof of application with applied date, required in packet at time of submission\n\nAdditional Information : IVS to OFFER Monday to Sunday Bi-weekly Orientations on Monday No block schedules for any reason This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit Please ensure all employment gaps over 30 days are explained Covid and Flu declinations accepted- please have either proof or declinations in packet \n\nDisclaimers : Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended Vehicles should be equipped with all season or snow tires during the months of November April
