Job Description
Provide habilitation services to each client as prescribed in that client's service plans (CFA, Individual Service plan, Habilitation plan)
Assist with activities of daily living (ADLs) such as preparing food, personal hygiene, and cleaning
Assist in maintaining a clean and safe living environment
Willingness to support people's preferences for community, activities, work, leisure activities, the exercise of their rights etc. in various settings
Other duties as assigned
Locations Available : Brooklyn, Bronx, Manhattan, Queens, and Westchester County
Shifts Available : Morning, Afternoon, and Overnight (Monday - Sunday)
Pay Rate : $15.00 / hr - $17.00 / hr. (weekly payment)
Degree Requirements
Qualifications
High School Diploma or GED required - If from another country, the Diploma must be evaluated
1-year previous experience providing one on one direct care required with the developmentally disabled population
Ability to stay alert and always awake; use a computer for data entry
Must obtain BLS (Basic Life Support) Certification, TB Testing, and OPWDD Orientation before starting work
Must be willing to assist with tasks that require lifting and standing for long periods of time
