Job Title: Licensed Practical Nurse (LPN)
Shifts: Night (11 PM - 7 AM), 8-hour shifts
Location: Champlain Valley Medical Center (CVPH), Skilled Nursing Unit, NY
Roles and Responsibilities:
- Provide care in a 34-bed Long-Term Care (LTC)/Skilled Rehab unit within an acute care setting.
- Deliver patient-centered care for a population aged 45-99 with varied diagnoses.
- Collaborate with CNAs and support staff to ensure optimal care delivery.
- Utilize Sigmacare for documentation and medication administration.
- Adhere to LTC/SNF protocols and assist with acute hospital needs as a sitter, if required.
- Maintain compliance with dress code and unit policies.
Education and Certification/License Requirements:
- Valid NYS LPN license (mandatory).
- BLS certification (required).
- Minimum of 2 years of Skilled Nursing Facility (SNF) experience preferred.
- Experience with Sigma Care documentation system preferred.
Salary : $2,000 - $2,130
