What are the responsibilities and job description for the Psychosocial Rehabilitation Paraprofessional position at Pride in North Carolina, LLC?
PSR staff provide services adults with developmental disabilities or mental health issues. Persons employed in these positions are expected to implement person centered plans designed specifically for each consumer. Employees are responsible for overall safety and well being of the consumers while providing services. As such, persons employed in these positions must be knowledgeable of the populations they are serving, must be able to plan and engage in therapeutic and skill building activities,provide supportive counseling, and be able to intercede with and de-escalate aggressive behaviors. A high school diploma is minimally required, however experience in the human service field is preferred.
GENERAL REQUIREMENTS:
Relationships with Service Recipients and their Families:
- Provides appropriate level of supervision to consumers. Abides by policies pertaining to supervision and allows unsupervised time only as specifically authorized per policy.
- Maintains a positive professional attitude and therapeutic relationship with consumers and families. Does not cross boundaries of professionalism with consumers or family members and does not develop personal relationships/friendships with consumers or family members. Abides by policies pertaining to professional conduct.
- Communicates appropriately with consumers and serves as a positive role model. Does not use profanity or any other inappropriate statements in the presence of consumers and does not smoke in the presence of consumers.
- Uses tact when discussing sensitive issues with consumers, families and guardians. Keeps supervisor informed of any treatment issues that arise.
- Demonstrates understanding of consumer rights and ensures rights are protected. Notifies supervisor of any possible consumer rights violations. Abides by all consumer rights policies and completes required training on consumer rights and confidentiality requirements.
- Respects and maintains confidentiality with regard to information concerning consumers and family members. Does not discuss confidential information outside of work or disclose any information at any time that is not necessary for treatment purposes.
- Responds appropriately during crisis situations; is able to defuse and de-escalate behavior outbursts. Is familiar with the crisis plan for each consumer and implements crisis plans as written. Abides by policies pertaining to emergency procedures.
Interventions and Service Provision:
- Maintains communication with the qualified professional and/or supervisor regarding consumer treatment or concerns. Understands and facilitates a person-centered approach to treatment.
- Implements interventions aimed at achieving individualized goals as written in the Person Centered Plan. Provides services and interventions in various locations, depending on the specific service and PRIDE IN NORTH CAROLINA, LLC. program
- Teaches problem solving skills and positive alternatives to inappropriate behaviors and coping skills. Provides consumers with clear consistent expectations and outlines potential consequences. Encourages consumers to take responsibility for their own actions.
- Plans, organizes and participates with consumer in therapeutic activities designed to improve emotional, vocational, educational, and social functioning. Ensures activities are age appropriate, promote cultural diversity, and are designed to meet treatment support needs.
- Teaches such skills as daily living skills, appropriate social skills, adaptation skills, health and wellness, development of leisure interests, work readiness, money management, budgeting, symptom management, etc
Documentation:
- Records accurate and informative service notes using correct format according to state and Medicaid guidelines, which reflect purpose of contact, interventions, time spent performing interventions, effectiveness, and include provider signature and credentials. Ensures notes are completed at the end of each shift.
- Enters service notes and other required documentation and information into information management system. Abides by policies pertaining to service records, computer use, and electronic maintenance of confidential information.
- Documents incidents into information management system and notifies supervisor immediately of any incidents. Makes any needed corrections to incident reports within 24 hours.
- This position is in our Psychosocial Rehabilitation program serving adults.
- Monday - Friday daytime position.
