Pride Manchester House has a very rare opportunity for a Licensed Social Worker to be a Case Manager for youth with behavioral needs in a psychiatric residential treatment facility.
This position is responsible for coordinating and overseeing the care and treatment of children (ages 5-13) within the Psychiatric Residential Treatment Facility (PRTF). The role involves managing incoming referrals, facilitating care conferences, and coordinating admissions and discharges. The Case Manager plays a key role in ensuring children and families receive trauma-informed care through family therapy, family modeling, and ensuring effective discharge planning. A key focus is on community reintegration and ensuring appropriate services are in place post-discharge. The Case Manager must adhere to Trauma-Informed Care (TIC) principles and foster an environment that prioritizes safety, empowerment, and collaboration. The Case Manager supports the development and implementation of care plans in collaboration with the clinical team.
1.Utilize trauma-informed, culturally sensitive approaches to support the well-being, emotional regulation, and autonomy of children and families. Continuously assess and manage emotional regulation to ensure a compassionate and effective approach in all interactions.
2.Provide case management services including referrals, treatment planning, discharge planning, and community-based education. Collaborate with families, external service providers, and community resources to ensure continuity of care and support post-discharge.
3.Collect and review outcome data for each youth to assess progress toward treatment goals. Collaborate with the clinical team to analyses this data and adjust care plans as needed. Submit necessary outcome data to the executive director in a timely manner. Adhere to all state and federal codes regarding data collection and program improvement.
4.Conduct ongoing assessments of the strengths and needs of children and families to inform individualized care plans, ensuring alignment with treatment goals and available resources.
5.Assist the executive director in the facilitation and organization of Admissions and Discharge meetings to determine appropriate placement and discharge
6.Assist the Team Coordinator in implementing groups and activities that are trauma-informed and support emotional regulations, social skills development, and coping strategies.
7.Lead discharge planning efforts and organize transition services for youth returning to the community, ensuring post-discharge needs are identified and referrals are made to community providers. Collaborate with the clinical team to ensure treatment plans are consistent and updated.
8.Lead multi-disciplinary care conferences, child-centered team meetings, and ensure active participation from internal staff, parents/custodians, and other stakeholders. Collaborate closely with the clinical team to ensure treatment plans are individualized, implemented, and consistently adjusted as necessary.
9.Maintain accurate and up-to-date records on referrals, admissions, discharges, treatment outcomes, and progress notes in compliance with organizational policies and licensure standards. Track and report treatment progress, ensuring adherence to documentation.
10.Model effective strategies for parents/custodians to empower their Childs' ongoing progress. Provide guidance and role-modeling to direct care staff and colleagues, fostering a positive, supportive, and trauma-informed environment.
11.Collaborate with other treatment providers to ensure consistent approaches to emotional regulation across all settings. Lead reflection sessions with youth to help process emotional outbursts or difficult situations and engage in reinforcing coping skills and emotional regulation techniques to build mastery and confidence.
12.Attend required training, including trauma-informed care, crisis intervention, and other relevant topics. Stay current on best practices in case management, therapeutic interventions, and child development. Engaging in reflective supervision to explore how personal responses may impact interactions and to support ongoing professional growth.
13.Ensure safety protocols are followed, particularly in high-risk situations. Support de-escalation efforts using Crisis Prevention Intervention (CPI) and approved trauma-informed techniques and interventions. Use non-judgmental language to avoid potential triggers and re-traumatization.
14.Engage in community outreach and foster partnerships with service providers, including schools, to strengthen support systems for youth and families. Collaborate with school staff to ensure consistency in care and the effective application of emotional regulation and coping strategies in the school setting.
Other Duties:
1.Adhere to facility policies and procedures, including confidentiality, ethical guidelines, and safety standards.
2.Perform additional duties as required to support the overall therapeutic treatment process.

SKILLS & ABILITIES
1. Strong understanding and experience with Trauma-Informed Care (TIC) principles.
2. Excellent interpersonal, communication, and organizational skills.
3. Ability to lead and facilitate case conferences, treatment planning meetings, and family therapy sessions.
4. Knowledge of behavioral health, child development, and crisis intervention techniques.
5. Ability to collaborate effectively with a multidisciplinary team.
6. Strong problem-solving skills and the ability to manage stressful situations

Experience
