Title Social Worker/RN Case Manager
Location: Tuba City AZ
Duration: 13 Weeks
Schedule: 40hrs./week
Shift: Monday to Friday, 8am-5pm Weekend on call rotation, would be 1 weekend a month
POSITION SUMMARY
The Social Worker/RN Case Manager is responsible for utilization review of patient cases in the inpatient and outpatient services departments, and review of medical necessity of referrals to specialty providers for services not normally provided within the facility. The incumbent will work with a multidisciplinary team to promote the efficient and effective use of a variety of health care resources by focusing on the whole patient to provide a seamless continuum of care. The goals are the provision of quality care, decrease the fragmentation of care, enhancement of patient self-care and quality of life, and cost containment.
Education:
MSW Social Worker:
License/Certification:
Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment
Licensed Master Social Worker (LMSW). A valid, current, full and unrestricted Professional License to practice LMSW or LCSW in any state of the United States of America, The Commonwealth of Puerto Rico, or a territory of the United States
Experience:
- Must have a minimum of three (3) years of experience in the clinical care setting
- Experience and knowledge of discharge planning, case management, or utilization review processes.
- Demonstrate knowledge of electronic health record systems.
