Summary Description
- We are looking for a Care Coordinator to enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle.
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
- Handle case assignments, draft service plans, review case progress and determines case closure.
- Facilitate multiple care aspects (case coordination, information sharing, etc.) with case managers and the insurance.
- Help client make informed decisions by acting as their advocate regarding their care giver and their care plan.
- Take the extra mile and interact with your clients to keep track of their progress and to ensure satisfaction.
- Record cases information, complete accurately all necessary forms and files as required.
- Supervise care givers: HR paperwork, schedules and training.
- Recruiting care givers through social media to ensure clients can be staffed
- Network with case managers and referral sources to meet goals and grow census.
- Adhere to professional standards as outlined by protocols, rules and regulations of Texas Medicaid PAS program.
- Proven working experience in home care , including as a care manager, care giver manager or a related job.
- Excellent knowledge of case management principles, healthcare management and Electronic Visit Verification.
- Effective communication skills.
- Excellent organizational and time management skills.
- Problem solving skills and ability to multitask.
- Current Valid Driver License the position requires travel to client homes with your own vehicle
