Our company was founded in 2005 and serves the Central Florida geriatric population. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. We are committed to providing excellent care to our patients and buildings while reducing readmissions into the hospital.
As an Advanced Practice Registered Nurse (APRN), you will play a crucial role in delivering high-quality healthcare services to patients in various settings depending on the location and care to be given. Your primary responsibility will be to assess, diagnose, and manage patient care, ensuring that each individual receives personalized treatment plans tailored to their unique needs. You will collaborate with a multidisciplinary team to enhance patient outcomes and promote health education within the community. This position requires a strong commitment to patient advocacy and the ability to navigate complex healthcare systems effectively. Minimum Qualifications:
- Master's degree in Nursing from an accredited program.
- Current APRN licensure in the state of SMPL.
- Certification in a relevant specialty area.

- Bilingual skills to enhance communication with diverse patient populations.
- Familiarity with electronic health record (EHR) systems.
