Job Description
I. Roles & Responsibilities
Overview: Ensures efficiency for PCSG as lead clinician of nursing services and clinical office environment management for the Blakely site. Works with front office staff and nurses to ensure acutely ill patients are seen as needed. Provides leadership in review of Quality Assurance/Community Health Center Measures Program. Coordinates community health education endeavors both internal and external to the organization.
Supervision, Education and Support of Clinical Staff
1. Systems and Equipment – Educate and support all clinical staff on the following:
· eClinical Works
· Lab Ordering System/Interface
· In-house lab testing/Point of Care Testing
· Use of clinical equipment
· Use of IT equipment needed to perform job duties
· Georgia Registry Immunization Tracking System (GRITS)
2. Quality Measures Training (includes UDS/ACO/HEDIS/Meaningful Use/PCMH) – Provide education to clinical staff on Quality Measures required by Primary Care and proper documentation to capture compliance.
3. Employee Education Training Program - Assist in the enhancement and implementation of the Employee Education Training Program which includes providing evidence based education to clinical staff (and non-clinical staff - applicable areas in bold) for the following areas:
· Risk Management (Blood-borne Pathogens & Safety Plans)
· Patient Centered Medical Home (Cultural Competency, Population Management, Self-Management including self-efficacy and behavior change, Communication Skills, Team Base Care, Medical Home Responsibilities, Care Coordination, Care Plans & Clinical Guidelines)
· Clinical Trainings (Clinical Policy and Procedures, Skills Evaluation, EMR Competencies, Peer Review, Standing Orders, Immunization and GRITS training).
4. Supervision
· Supervises all assigned non-provider clinical staff.
· Serves as a direct manager of clinical flow and clinical operations.
5. Support
· Assist Remote IT and eClinical in resolving issues with hardware and software to support staff productivity.
· Provides direct patient care as back up when needed for the health center.
· Works as a team player with providers and other medical personnel to assure clinic success.
· RNs perform frequent patient evaluations, including triage, monitoring and tracking vital signs, performing procedures such as IV placement, phlebotomy, and administering medications.
· Other duties as assigned.
6. Patient Education - Conducts patient education, organizes educational materials. Plans and implements patient education classes in collaboration with Health Educator, when one is employed.
Policies, Procedures and Guidelines - Implements, monitor and provide oversight of policies and procedures to provide quality assurance for the following policies and procedures. In addition to providing input for updates needed.

Evaluate Performance/Quality Reporting
A. Clinical Quality Improvement – Through the Clinical Quality Improvement Committee Meeting
· Monitors and reports on Quality Measures by individual clinician/team and results across the practice.
· Responsible for the coordination and monitoring of clinical outcomes data towards UDS reporting and Office of Performance Review.
· Collects and reports on Quality Data to staff, providers and reporting agencies. Quality Data Reports Include:
§ NAP Grant/UDS Measures
§ ACO Measures
§ Meaningful Use Measures
§ HEDIS Measures
§ PCMH Standards
§ AAAHC Standards
B. Performs Annual Performance Evaluations on non-provider Clinical Staff in collaboration with the teams lead provider. Evaluations are signed by the CEO and CFO and filed with Human Resources.
C. Competency Evaluations – Performs Clinical Skills Competencies and EMR Documentation Evaluation on non-provider staff annually.
D. Compliance reports - Generate compliance reports weekly by provider team for outstanding labs, test, referrals, telephone encounters and unlocked visits. Follows-up with team to resolve issues.
Scheduling Coordination
1. Patient Scheduling
· Works closely with Business Office staff regarding patient care and appointments.
· Monitors eClinical Schedule for patterns of heavy No-Shows and seeks to identify process related issues to decrease No-Shows.
· Monitors the utilization of Same Day Appointments to assess the need for increasing or decreasing the amount available
2. Clinical Staff Scheduling
· Manages Clinical Work Schedule for staff coverage.
· Maintains a back-up staff coverage plan to cover assignments for staff on leave.
· Approves PTO for non-provider Clinical Staff in collaboration with Human Resources providing employee has enough PTO for time off and the schedule permits time off.
· Coordinates staff lunch times to minimize negative patient impact and overtime.
· Monitors staff time for overtime and make provisions to avoid overtime.
3. Providers
· Notifies Site Manager if there is a need for scheduling changes. The Site Manager will obtain permission from CEO, CFO or COO and alter or block schedules, if approved.
· Blocks Provider schedules for holidays and meetings in eClinical. All PTO blocks will be instructed from the Site Manager once the CEO or CFO has approved.
· Coordinates After-Hours Coverage with Providers and assures the Call Service knows the correct contact information and method of contact. Adjustments needed at the Call Service level are made by the COO. Will facilitate changes through the COO.
4. Meetings · Responsible for ensuring staff attend all meetings (Clinical Staff Meetings, Provider and Clinical Staff Meeting, Front Office meetings and Employee Trainings that employee is organizing).
· Assures that the Provider Team (Provider, Primary Nurse/MA, and Scheduling Coordinator) is aware of schedule changes to contact patients in a timely manner for any reschedules.
5. Drug Representative/Agencies Lunch Meetings – Coordinates lunch meetings and places on Outlook Calendar.
Resource Management/Waste
1. Medical Supply Management
· Assesses weekly clinical inventory and orders supplies as needed.
· Contacts Director of Nursing for non-standard purchases.
· Compares prices for the best economical choice in products but seeks council before changing a routinely purchased item.
· Complies with the Purchase Order system.
· Monitors numbers of encounters each day to support budgeted number of encounters.
· Ensures that the health checks and other services are monitored to improve clinic volume of patients.
2. Clinical Equipment - Maintains updated list of clinical equipment and request repairs as needed.
3. Medications/Inventory
· Maintains/delegates maintenance of sample medication inventory, donated medications (Direct Relief) and donated immunization inventory.
· Returns expired immunizations for account credits within the appropriate time frame.
· Disposes or delegates the disposal of expire medications per guidelines.
4. Bio-hazardous Waste – Packages or delegates the packaging of bio-hazardous waste per guidelines for Stericycle pickup.
Meeting Participation and Committee Activities
1. Clinical Quality Improvement Meetings and Role
· Attends all QI meetings.
· Monitors ongoing effectiveness through evaluating data in the Clinical QI Meetings that track results over time, access the effects of its action and promote achieving improved performance on measures.
· Assist in establishing/identifying important conditions and data for list to send reminders for needed services.
· In conjunction with the CQI Committee and through the QI process assist in the following:
§ Setting goals, acting to improve performance on measures and identify disparities to improve for vulnerable populations.
§ Assesses racial, ethnical and language diversity to determine any changes in needed services.
· Assist in stratifying data for QI reporting.
· Assists Quality Manager in the preparation of QI results across the practice to display on bulletin boards in the lobbies and website.
2. Provider and Clinical Meetings and Role – Attends, prepares information for meeting and setup.
3. Risk Management Meetings and Role
· Evaluates and coordinates management of medical emergencies and prepares post evaluation for Risk Management.
· Provides role of Clinical Risk Manager/ Infection Preventionist in assessing for clinical and infection risk and provides feedback to Risk Management Committee; educates staff and implements infection control methods. Schedules annual Infection Control Training for staff.
4. Community Coordination
· In Collaboration with the Outreach and Enrollment Specialist, facilitates work with outside groups; i.e. schools, other providers of health and social services, to assist with promotion of health center services and growth of patient base.
· Maintains a current resource to key community service areas of importance to the patient populations.
· Assist with tracking frequency and type of referrals to community agencies to evaluate appropriate resources.
· Establishes and maintains relationships with community organizations, health plans, facilities and specialist.
· Promotes harmonious relationships and favorable attitudes among the health care team, patients and public.
G. Facilitates relationships with other agencies (ie. lab and diagnostic imaging) to establish/maintain electronic exchange of information.
H. Facilitate clinical data exchange to other entities, immunization registries and syndrome surveillance data to public health.
I. Employee Health/Records – Coordinates/delegates annual employee health screens, PPDs and immunizations. Completes forms for Human Resources records.
II. Operations Manager Responsibilities
· Oversees clinic management and supports the COO of PCSG in implementing federal rules and regulations.
· Performs a wide variety of executive administrative duties as required by daily operations to include assuring smooth operations of the site, reporting maintenance issues of building to appropriate Supervisor and other assignments.
· Assures staff is aware of all meetings; assures room for meeting is reserved and equipment is set up as needed.
Duties and Responsibilities
· Works with Risk Manager to assure patient satisfaction surveys are conducted two times a year.
· Assists HR Coordinator with compiling Credentialing documents of healthcare professionals and tracking of CME documentation
· Assist HR Coordinator in compiling Employee Satisfaction surveys annually
· Works in a financial counseling capacity to assure patient advocacy
· Facilitate resolution of issues with patient access and facility needs
· Works in a financial counseling capacity to assure patient advocacy
o Reviews uninsured patient’s eligibility
o Assures referral to Outreach Worker for Medicaid or other plan enrollment forms for qualified individuals
o Counsels patients on outstanding bills and educates on options for payment
· Sets up for meeting when needed.
III. Qualifications & Education Requirements
Graduate from an accredited school of nursing with LPN or RN credentials. Must maintain a current LPN or RN license to practice in the State of Georgia. Nursing and management experience required. Computer literacy. Exceptional leadership and communication skills.
