Integrated Behavioral Health Clinician
Job Summary:
The clinician will work in collaboration with primary care providers across the practice clinics. This includes work with individuals who are diagnosed with behavioral health conditions as well as individuals who experience difficulty in managing health conditions. Interest in clinical work from a bio-psycho-social model, including consideration of the psychosocial aspects of chronic and acute diseases. The clinician will work as a member of the integrated behavioral health team via a stepped care approach to improve patient functioning using modalities appropriate in a primary care setting.
Duties/Responsibilities:
- To bring behavioral health integration to patients and their families in the primary care setting.
- To improve population health outcomes and patient functioning.
- To co-manage patient concerns with the care team.
- To include evidence-based screening and diagnostic assessments, brief targeted interventions, and referrals to specialty resources as necessary for integrated behavioral health clinical services.
- To continue to develop a working knowledge of chronic disease management interventions, patient motivations to work toward change, and psycho-pharmacology.
- To collaborate with healthcare teams on developing common treatment modalities used in primary care settings (e.g., cognitive behavioral therapy, behavioral activation, motivational interviewing, relapse preventions, clinical case management, informal medication monitoring).
- To conduct individual appointments 20-45 minutes, group medical visits, consultation, and warm hand-offs (in exam rooms, curbside, and team treatment planning) in their work formats.
- To facilitate mindfulness-based groups for topics such as: chronic pain, eating, patient empowerment, substance use/addiction, parenting groups.
- To document services in a shared electronic medical record that facilitates team communication and care planning
Work Experience:
Preferred a minimum of 2 year experience in health care environment.
Education Requirements:
Licensed Clinical Social Worker, Licensed Counselor or Licensed Marriage and Family Therapist
General Requirements:
Strong problem solving and comfort with the inherent fast pace of primary care medicine.
Working Conditions:
Works in a well-lighted and ventilated office; sitting, standing and walking; light to moderate lifting occasionally

Position Type/Expected Hours of Work:
This is a full-time exempt position 40 hours or more a week. Primarily works Monday through Friday position 8 a.m. 4:00 p.m.
