Medical Scribe LPN/RN
Job Summary:
Provide basic and expanded patient care roles, nursing tasks, and education under the direction of the physician.
Assume independent responsibility for patient immunizations, ordering preventive and chronic disease tests or procedures by protocol, take an active role throughout the patient visit, scribing entirety of the encounter into the patient record; enacting patient care needs or diagnostics during the course of the visit, and summarizing the encounter for the patient in appropriate manner - verbal /written format.
Duties/Responsibilities:
Insurance Auditing
- Work with management to understand the metrics and gap closures needed for each insurance
- Work together with Care Coordinators and Management to determine work flow and to develop solutions to new problems that arise
- Work independently on gap closures
Metric Management
- Call and schedule patients that are due for specific appointments (i.e., DM or HTN appointments)
- Send letters to patients that are due for Pxs, WCCs and/or AWVs weekly.
- Ensure that patients who have been sent letters are actually scheduled in a timely manner, utilizing spreadsheet and ticklers when required.
Patient Care
- Obtain and chart vital statistics
- Provide patient instruction and education materials
- Make and chart appointments for out-of-office exams as well as in office appointments
- Prepare, administer and chart medications and injections
- Perform skills from checklist
- Point of Contact tests: EKGs, PFTs, A1cs, swabs, pelvic assists, vision screens, injections as needed
- Fill in when MA is out.
- Order various procedures and labs as directed by physician or practice protocols.
- Call test results to patients
Assisting Physician
- Chart Maintenance - Review /reconcile medications, resolve acute meds and problems, expired/overdue orders
- Review patient specific alerts/reminders, and act on them per protocol or in communication with physician
- Assist physicians with specialized minor procedures
- Keep physician informed on his daily schedule
- Perform functions according to physician orders
- Dispense medications at physician direction
Expanded MA/nurse/scribe role
- Gather CC: and goals for the visit
- Paraphrase/Document patient's pertinent history, and treatment plans in the EMR - Scribing the narrative HX, ROS, PE and PLAN
- Document the PLAN section of encounter , so it is available to patient summary upon completion of visit, and opportunity to print and review with patient
- Enter Orders, Meds, Refills, Referrals as discussed during the encounter
- Where workflow permits, print and review patient education, clinical summary, and relevant printed/on screen material with the patient
- Facilitate follow up/continuity with patients with proactive monitoring for test results, and communication/verification of results with the physician and patient as they become available
- Assume independent responsibility for Review and managing patients' Immunization, and Preventive health needs, and act on them independent of the provider
Administrative
- Complete encounter form
- Clean and supply patient rooms
- Autoclave and wrap used instruments, fill in when Technician is out of office.
Experience:
Must be a Medical Assistant or Certified Medical Office Assistant, LPN or RN, who is capable of working back office position, caring personality with a positive attitude, calm under pressure and exhibit sound judgment, excellent time management and problem solving abilities, and good communication skills. Must have knowledge of the proper use of medical and lab equipment. Previous medical office experience preferred.
Education Requirements:
Have appropriate certification or licensure. Attend Continuing Education programs as requested or as required by the position.
Working Conditions:
Works in a well-lighted and ventilated area; primarily standing and walking; light to moderate lifting all the time; heavy lifting occasionally.

While performing the duties of this job, the employee is regularly required to talk and hear. This is both a sedentary and walking/standing role. This position requires a significant amount of typing and computer screen viewing. Specific vision abilities required by this job includes close vision, distance vision, and ability to adjust focus.
Position Type/Expected Hours of Work
This is a full-time position of 40 hours. Days and hours of work Monday through Friday, 8:00 am to 4:00 pm. Expected hours of work may change at any time with or without notice.
