Job description

I/DD Qualified Professionals are expected to deliver quality care to mentally or emotionally disturbed and/or developmentally disabled clients that meet all the other program requirements outlined under NC Innovations Waiver Manual. The North Carolina Innovations Waiver is a resource for funding services and supports for individuals with intellectual and other related developmental disabilities that are at risk for institutional care in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF-IDD). NC Innovations is designed to provide an array of community-based services and supports to promote choice, control, and community membership. These services provide a community-based alternative to institutional care for persons who require an ICF-IID level of care and meet additional eligibility criteria for this waiver.

Duties and Responsibilities:

  • Participating in person-centered planning for the development of the Individual's Support Plan, including revisions with the Care Coordinator and the person/legally responsible person.
  • Recruiting appropriate personnel.
  • Conduct Supervision Reports to staff.
  • Interview and supervise direct care providers of services per General Statute 10A NCAC27G.0101.
  • Ensuring personnel are trained and supervised in the provision of services.
  • Ensuring back-up staff is available when the lack of immediate care would pose a serious threat to the individual's health and welfare and formal providers are unavailable. If back-up staff is unavailable, the provider agency must complete a Level 1 Incident Report and submit it to the Quality Management department.
  • Implementing the services authorized in the Individual Service Plan.
  • Provide monitoring of direct care staff in the implementation of modalities and interventions.
  • Provide documentation training, incident report training, treatment modalities and interventions, medical records and other required training.
  • Provide intervention.
  • Daily and/or weekly review/approval of documentation of service notes by direct care staff as per the requirements and standards of Continuous Quality Improvement (CQI) to assure all the components of the individual's chart are accurate.
  • Assisting in the coordination of services and communication with the individual/family.
  • Monitoring services authorized by service orders to ensure consistently with the Individual Support Plan.
  • Serves as a link between individual, community agencies and family.
  • Demonstrate knowledge, skills and abilities by the population served.
  • Review and maintain adequate documentation, as required; making it available to Care Coordinators, the MCO and others as needed.
  • Monitors and reviews services in all settings that include the review of the documentation of direct care staff/contract providers.
  • Flexible and ready for any occurring changes.
  • Maintain good working relationship with family and consumer.

Job Type: Full-time

Pay: $53,000.00 - $65,000.00 per year
