Job Summary:
We are seeking a Qualified Professional to join our team. The ideal candidate will be responsible for patient assessment, case management, and providing counseling services to youth and their family. Intensive In-home Services require a team approach designed to address the identified needs of children and adolescents who are transitioning from out of home placements or are at risk of out-of home placement and need intensive interventions to remain stable in the community.
Intensive In-Home Services are to be provided to stabilize the living arrangement, promote reunification or prevent the utilization of out of home therapeutic resources. Services are time-limited intensive family preservation intervention and are delivered primarily to children in their family’s home through the age of 20.
Duties:
- Conduct patient assessments to determine needs and develop treatment plans
- Provide case management services to ensure continuity of care
- Utilize motivational interviewing techniques to support clients in achieving their goals
- Maintain accurate daily medical documentation in compliance with regulations
- Implement art therapy as a creative approach to healing
- Collect and analyze data to monitor progress and outcomes
- Manage behaviors through effective strategies
- Offer child and family counseling services
- Conduct individual and group counseling sessions
- Resolve conflicts and facilitate conflict management
- Be able to assist the consumer and family to diffuse current crisis situation and reduce recurrence
- Ensure linkage to needed community services and resources
- Provide self-help and living skills training for youth
- Provide parenting skills training to help the family build skills for coping with the youth’s disorder
- Monitor and manage the presenting psychiatric and/or addiction symptoms
- Work with caregivers in the implementation of home based behavior supports
- Provide more intensive services at the beginning of treatment and decrease over time as the youth and family’s coping skills develop
- Deliver structured and face-to-face services to provide support and guidance in all areas of functional domains
- Accepts supervision/training to enhance skills and knowledge in order to have and maintain a working knowledge in relation to job, therapeutic intervention and treatment modalities
- Responds to crisis situations when requested
- Attends treatment team meetings and other meetings as required
- Adheres to and follows all policies and procedures of this agency
Qualifications:
- Bachelor's degree in the human service field required
- Proven experience in patient assessment, case management, and counseling
- Strong knowledge of medical documentation practices
- Ability to collect and interpret data effectively
- Skilled in behavior management strategies
- Experience in child and family counseling
- Proficient in individual and group counseling techniques
- Excellent conflict resolution skills
