Responsibilities:
- Coordinates health care referrals by giving the referral consultant or specialist the clinical reason for the referral & pertinent clinical information.
- Tracks the status of referrals, including required timing for receiving a specialist’s report.
- Follow-up to obtain a specialist’s or consultant’s report.
- Records pertinent information in medical records. Duplicates and collects printed materials.
- Establishing & documenting agreements with specialists in the medical record if co-management is needed.
- Asking patients/families about self-referrals & requesting reports from clinicians.
- Receives and routes phone calls to appropriate personnel and takes accurate and complete messages. Responds to requests for assistance and/or general information. Greets visitors and provides requested information. Assures clinic preparation and follow-up of all programs.
- Collects information for statistical reports. Reviews records for accuracy and completeness. Types various reports, correspondence, records, case histories, etc.
- Deals with parties in emergency or delicate situations with tact and diplomacy. Locates numbers and provides general information to callers.
- Able to perform electronic exchange of the patient’s electronic medical record information between clinicians.
- Updates records with any and all changes in address, phone, income, household size, etc. each clinic visit. Able to provide an electronic summary of the patient’s record to another provider for more than 50 % of referrals.
- Identifies patients with a hospital admission & patients with a hospital an ER visit by checking the provider call log & following up with the patient/hospital.
- Share clinical information with admitting hospitals & ER departments & obtain patient discharge summaries from the hospital & other facilities.
- Exchanges patient information with the hospital during a patient’s hospitalization
- Complies with applicable law and fully compliant and cooperate with clinic's Compliance Program.
- Other duties as assigned.
Qualifications:
- Must be at least 18 years of age.
- Must be a licensed practical nurse or certified medical assistant.
- The ability to relate with effectiveness to the public, the patient, and the health care providers.
- The ability to effectively perform the duties that are part of the clinical activities, including the ability to perform basic mathematical functions consistently and correctly.
- Case management experience preferred.
Job Type: Full-time
Pay: $18.00 - $24.00 per hour
Expected hours: No less than 40 per week
