About the Role
">- You will assist patients with routine tasks, such as answering questions, resolving minor difficulties, and providing directions as authorized by the Provider.
- Document all patient communication in patient charts, including any Provider-directed instructions.
- Support patients through education and information on community resources, such as classes, support groups, and pharmacy assistance programs.
- Provide education and support to patients and families as directed by the Provider.
- Assist patients in basic self-management goal setting, applying active listening and behavior modification principles in goal discussions with patients.
Responsibilities
">- As directed by the Provider, assist with routine office examinations and procedures, including administering injections, medications, vaccinations, and blood draws.
- Document all procedures in the patient's chart.
- Document all patient interaction, track, and attach laboratory and diagnostic test results to patient charts; communicate results and orders to patients as directed by the Provider.
- Serve as a liaison to pharmacies, following up with patients as needed.
