We are seeking a compassionate and detail-oriented Case Manager to join our team and support at-risk individuals by connecting them with essential psychosocial services.
Position Summary
Under general supervision, the Case Manager will advise and assist individuals and families facing challenges such as personal and family adjustments, financial difficulties, employment concerns, housing instability, physical and mental health issues, and more. The Case Manager will conduct client assessments, develop service plans, and refer clients to appropriate community resources.
Key Responsibilities
- Conduct interviews with clients to assess their needs and determine the necessary support services.
- Gather and evaluate medical, psychological, and social information relevant to the client’s situation.
- Develop service plans to assist clients in achieving stability and improving social functioning.
- Support clients in modifying behaviors and developing coping strategies.
- Refer clients to community organizations and resources for further assistance.
- Maintain accurate and confidential client records in compliance with HIPAA regulations.
- Collaborate with healthcare providers and other professionals to ensure holistic care.
- Assist Behavioral Health providers as needed.
- Adhere to organizational standards, including Continuous Quality Improvement, Risk Management, and Patient-Centered Medical Home initiatives.
Qualifications
- Education: High school diploma or GED required; Bachelor's degree in Social Work or a related field preferred. Relevant work experience may be considered in lieu of a degree.
- Experience: Knowledge of social work ethics, human development, and psychological interventions in healthcare settings.
- Skills:
- Strong verbal and written communication skills
- Ability to work with individuals, families, and small groups
- Critical thinking, problem-solving, and active listening skills
- Strong organizational skills and ability to work in a fast-paced environment
- Ability to maintain professional boundaries and demonstrate empathy
- Technical Proficiency: Intermediate computer skills preferred, including familiarity with Microsoft Office (Word, Excel, PowerPoint, Publisher).
- Other Requirements:
- Valid driver’s license required
- Must comply with all current CMS vaccine mandates
Work Environment & Physical Demands
- Ability to sit, stand, walk, bend, and stretch for extended periods.
- Occasional lifting of up to 50 pounds.
- Frequent travel between clinical sites in Northwest Georgia may be required.
- Exposure to common viruses and conditions in a community healthcare setting.
- Ability to work flexible hours and manage stressful situations.
