Summary
We are looking for a compassionate and dedicated Care Worker to join our team at Primary Home Health Care Inc. This vital role involves providing personalized support to clients in their homes, ensuring their comfort, safety, and overall well-being.
Responsibilities
- Assist clients with daily living activities such as bathing, dressing, and meal preparation.
- Monitor and record vital signs and report any changes in health status.
- Provide memory care support for clients with cognitive impairments.
- Administer medications as prescribed and ensure compliance with care plans.
- Assist clients with mobility and heavy lifting when necessary.
- Maintain a clean and safe environment for clients.
- Support individuals with disabilities by promoting independence and dignity.
Qualifications
- Basic knowledge of general housekeeping tasks.
- Experience in Home Health is preferred, but we will train you.
- Be at least eighteen (21) years of age.
- Demonstrated ability to read, write, and follow a written Plan of Care.
- Good verbal, written, and interpersonal skills.
- Licensed driver with automobile insured following state and/or Agency requirements.
- Reliable transportation, reasonable driving record, and willingness to commute within the Colorado Springs and Fountain area to client's homes to provide care.
Description
- Perform procedures as an extension of nursing and therapy services under the direction and supervision of therapists or RNs.
- Assistance in ambulation or exercises
Performs personal care activities, including but not limited to:
- Bathing
- Shampooing
- Skincare/nail care
- Oral hygiene
- Shaving
- Peri-care
- Dressing
- Performs household services essential to health care at home, including but not limited to:
- Meal preparation/feeding
- Laundry
- Light housekeeping
- Errands, shopping, and prescription pick up
- Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse. Follow reporting guidelines in the Care Plan.
- Document care provided and complete the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submit documentation per Agency requirements.
- Promotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
- Demonstrates safe practice in the use of equipment. Do not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
- Maintains confidentiality in all aspects of the job.
