PRimary Home Health Care Inc. is a Registered Nurse owned Home Health Care Agency seeking a Registered Nurse, Case Manager, to join our team! Excellent clinical skills, and knowledge of Medicare/Medicaid regulations, and home health experience preferred.

Job Description:
- Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
- Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
- Communicates significant changes in client status to physician and other members of the team regarding the continuity of patient care.
- Collaborates patient care with auxiliary team members.
- Maintains client records showing systematic assessment, planning intervention and evaluation.
- Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations.
Qualifications:
- Current Registered Nurse license in the state of Colorado.
- CPR Certified
- Strong verbal, written and interpersonal communication skills.
- Demonstrates time management, organizational skills, and ability to function with minimal direction.
- Valid driver’s license, car insurance and reliable transportation.
- Home Health experience
- Applicants must meet all federal & state mandated vaccination, testing, and screening requirements. Ie: covid-19, influenza, and TB
- Must be able to work at least two days a week, with Home Health experience
