Targeted Case Manager - Behavioral Health
Primary Purpose: As a Targeted Case Manager in the field of Behavioral Health, your primary responsibility will be to provide support and assistance to individuals in need of mental health services. Education and Experience:
- Bachelor's degree in social work or a related human services field is required.
- Possess a current driver's license.
Skills and Abilities:
- Proficient in basic computer skills and the ability to complete and submit reports.
- Self-motivated and able to work independently while managing a caseload.
- Excellent documentation skills with attention to detail.
- Strong written and verbal communication skills for effective interaction with clients, colleagues, and stakeholders.
- Adept in utilizing search engines and other digital resources for research and information gathering.
- Capable of handling crisis situations calmly and providing support to individuals in distress.
- Maintain a positive and professional attitude towards clients and staff.
Responsibilities: As a Targeted Case Manager, you will be responsible for the following tasks:
Confidentiality and Compliance:
- Maintain strict confidentiality of client information and adhere to company, state, federal, and HIPAA regulations.
- Ensure accurate and timely documentation for insurance purposes.
Client Support:
- Foster a safe and healthy environment for clients, promoting their overall well-being.
- Demonstrate a sincere commitment to each client's recovery journey.
- Monitor clients' emotional well-being and behavior, identifying any changes or concerns.
- Transport clients to appointments when necessary, ensuring their attendance and access to required services.
- Connect clients with family and community resources that support their mental health needs.
- Coordinate with legal entities when necessary to ensure clients' legal rights and obligations are met.
Case Management:
- Motivate clients towards goal achievement, providing guidance and support.
- Conduct weekly meetings with clients to assess progress, address challenges, and update care plans.
- Assist residents with long-term care plan development, ensuring their individualized needs and goals are addressed.
- Provide educational resources and support to clients, helping them gain knowledge and skills related to their mental health and well-being.
