Job Summary: Primary Vascular Care is seeking a highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments and develop personalized treatment plans for residents of client facilities.
The ideal candidate will work independently in a mobile practice, primarily serving long-term care settings. This role requires strong assessment and communication skills, as well as the ability to maintain relationships with facility contacts and educate clients and families about services offered by Primary Vascular Care.
Essential Functions:
- Perform on-site screening clinical assessments to determine appropriate patient treatment plans, completing comprehensive assessment forms
- Function independently to perform history and physicals for complex acute, critical, and chronically ill patients
- Order and review diagnostic and therapeutic tests relative to patient needs
- Assist with provision of care according to facility, state, and federal regulations
- Maintain accurate records and monitor intervention effectiveness
- Maintain relationships with facility contacts and other relevant parties
- Educate clients, families, and prospective accounts about services offered by Primary Vascular Care
- Collaborate with physicians to develop and maintain professional relations
- Complete timely and thorough reports as assigned
Qualifications:
- Bachelor's degree from an accredited institution
- Degree from an accredited Nurse Practitioner or Physician Assistant Program
- Current NP license
- BLS certification through American Heart Association
- ACLS certification through American Heart Association (preferred)
- Infection Control Certification
- Board certification or eligibility
- Completion of required vaccinations, including PPD/TB, Hep B, MMR, Varicella, and COVID-19/Flu vaccines (preferred)
- Minimum 1 year of NP experience (preferred)
