In this role, you'll help patients regain independence and improve physical and cognitive functioning by developing and implementing personalized treatment plans. This position includes comprehensive patient and caregiver education, regular progress updates, and collaborative work with other healthcare professionals.
Key Responsibilities :
- Perform Physical Therapy assessments and create individualized treatment programs tailored to each patient's needs.
- Record and report on patients' progress, adjusting treatment plans to achieve therapeutic goals.
- Educate patients and caregivers on therapy routines and safe equipment usage to foster ongoing health improvements.
- Supervise Physical Therapy Assistants and, when necessary, Certified Nursing Assistants, ensuring high-quality care delivery.
- Prepare detailed reports and actively participate in staff meetings and development activities within the Agency.
