Responsibilities
As a Case Manager RN, you will oversee the quality and resource management for all patients admitted to the facility, ensuring a smooth transition throughout their care journey. You will advocate for patients while promoting cost-effective practices and demonstrating leadership to enhance collaboration among healthcare providers. Your role will focus on facilitating quality clinical outcomes through thorough, holistic admission and ongoing reviews of medical records, assessing medical necessity, service intensity, and severity of illness.
