What are the responsibilities and job description for the Speech Language Pathologist position at Prime Home Developmental Disabilities Services?
Our agency is growing and we are adding a Speech Therapist to our team.
JOB SUMMARY: To provide optimal speech, language, communication care to clients. Demonstrates good problem-solving skills and ability to adapt without difficulty.
Responsibilities:
- Work closely with patients, families, and physicians to provide a personalized care experience by assessing individual’s abilities and needs.
- Initiate and revise care plans to meet patient goals.
- Maintain records and ensure care provided enhances the lives of our patients and meets state and federal guidelines.
- Provides evaluations of auditory, communication, developmental, and academic skills for infants, children, and adults with hearing loss or disorders of communication, feeding, or swallowing.
- Consults with medical, dietary, clinical, educational personnel, families, and other staff, including officers from other agencies as needed to ensure excellence in patient care.
- Prepares and maintains reports/records on patients.
- Participates in the development and implementation of training programs/materials for families and professionals.
- Assists with identified quality improvement initiatives by monitoring records of individuals served in clinical and educational programs.
- Provides patient care and education according to the patient population served.
Qualifications and Skills:
- Graduated from an approved Speech Language Pathologist program with a master’s degree or Doctorate of Speech Language Pathology.
- Possession of, or eligibility for, appropriate state licensing required.
- Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required.
- A current CPR Certification.
- A valid driver’s license, acceptable driving record and access to reliable transportation.
- Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required.
