Hiring Transitional Assistance/Community Support Specialist for Fall River and surrounding areas.
- Applicant must enjoy working with the frail and/or elderly in the home care setting.
- Must have excellent communication and documentation skills and be highly organized.
- Goal of the Community Support Specialist is to participate in establishing a safe, comfortable household in the community.
- Job requires staff to attend meetings with client, family, case manager and facility staff. Assist with purchase or provision of goods and services as clients transition from a facility to in home care. Other duties include the arrangement of transportation, shopping for household goods, viewing and applying for potential housing, touring prospective day programs and finding community medical providers.
- Additional tasks may include assisting clients with applications for SNAP and other state benefits, scheduling and attending MD appointments, assisting with budgeting and shopping.
- Experience with Transitional assistance and housing is preferred but similar experience in the community will be considered.
- Bachelor's or licensure in a related field preferred.
All cases are in the southeast MA area. The position entails scheduling flexibility as it is part remote. Current hours available are considered part time but are expected to increase to full time very soon.
Requirements:- Bachelor’s degree or licensure in a related field preferred, but equivalent community experience will be considered
- Preferred experience in transitional assistance and housing support.
- Experience in community services, case management, or related fields.
- Passion for working with frail and/or elderly individuals in a home care setting.
- Exceptional communication skills for interacting with clients, families, and other professionals.
- Strong documentation and organizational skills.
- Proficiency in assisting with applications for state benefits such as SNAP.
- Ability to assist with budgeting, shopping, and finding community resources such as medical providers and day programs
- Competence in attending meetings and coordinating services with clients, families, case managers, and facility staff.
- Ability to organize and facilitate client transitions from facilities to home care.
- Comfortable with arranging transportation, viewing housing, and scheduling appointments.
- Flexibility to work part-remote and adapt to a fluctuating schedule.
- Availability to work part-time initially, with the potential for full-time hours soon.
- Willingness to travel within the southeast Massachusetts area
- Reliable transportation for traveling to client locations.
- A commitment to building safe, comfortable, and supportive household environments for clients transitioning to home care.
- Bilingual in English and Spanish are highly encouraged to apply.
