Job Summary :
We are seeking a dedicated Registered Nurse (RN) – Home Care Case Manager to join our team. The RN will be responsible for client admissions, development of care plans, and ongoing supervision of home care services. This role involves oversight of Home Health Aides (HHA), Personal Care Assistants (PCA), Companions, and Homemakers , ensuring that care is provided according to the individualized care plan. The RN will also conduct scheduled revisions of care plans and may perform wellness skilled nursing visits for specific referral sources. This position does not require OASIS documentation.
Key Responsibilities :
- Conduct client admissions, including initial assessments and development of personalized care plans for non-Medicare clients.
- Create and adjust plans of care based on client needs, focusing on HHA, PCA, Companion, and Homemaking tasks.
- Provide ongoing supervision of caregivers, ensuring compliance with care plans and best practices.
- Perform scheduled care plan revisions and client reassessments as needed.
- Conduct wellness skilled nursing visits for designated referral sources as required.
- Collaborate with families, caregivers, and referral sources to ensure continuity of care.
- Maintain accurate and timely documentation of assessments, care plans, and supervisory visits.
- Ensure compliance with state regulations, agency policies, and patient care standards .
- Serve as a resource for caregiver training and education , ensuring aides are well-prepared to meet client needs.
