Join Our Compassionate San Antonio Team: Hospice Nurse Practitioner
Are you a dedicated and compassionate Nurse Practitioner with a passion for providing exceptional end-of-life care? We invite you to consider joining the growing, supportive and dynamic hospice team at Pacific Hospice and Palliative Care on a PRN basis, where you’ll have the opportunity to make a profound difference in the lives of patients and their families during their most critical time. If your schedule is flexible and you are able to see patients during weekdays, weekends and evenings, we may have several visits each week for you. These are in home face-to-face visits with patients, there are no telehealth visits, so you are required to live in the San Antonio area. Please do not apply if you do not live in the area.

Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for patients with terminal illnesses.
- Manage and coordinate patient care in collaboration with physicians, nurses, social workers, and other healthcare professionals.
- Provide symptom management and palliative care to enhance the quality of life for patients.
- Educate and support families and caregivers, offering guidance on end-of-life issues and emotional support.
- Participate in on-call duties and respond to patient needs as required.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications:
- Master’s Degree in Nursing (MSN) with a specialization as a Family Nurse Practitioner.
- Current NP licensure and certification in the State of Texas
- Minimum of 5 years of experience in hospice, palliative care, or a related field.
- Bilingual Spanish/English a plus.
- Strong clinical assessment skills and the ability to work independently.
- Experience with Axxess EHR or other EHR system and apps such as Slack for internal communication purposes
- Excellent communication and interpersonal skills, with a compassionate and empathetic approach to patient care.
- Ability to handle sensitive and emotional situations with professionalism and care.
