We are seeking a compassionate Speech Therapist to join our home health team on a PRN Part Time basis. The Speech Therapist will work with patients in their homes to assess, diagnose, and treat speech, language, and swallowing disorders. This role requires a deep commitment to improving the communication and quality of life of our patients, particularly those facing challenges due to illness, injury, or aging.
Responsibilities:
- Conduct comprehensive assessments to evaluate patients' speech, language, voice, and swallowing disorders.
- Develop and implement individualized treatment plans based on assessment findings and patient needs.
- Provide speech therapy interventions to improve communication skills, articulation, fluency, and voice quality.
- Administer and interpret standardized tests and evaluations to monitor progress and adjust treatment plans as necessary.
- Collaborate with a multidisciplinary team of healthcare professionals, including physicians, occupational therapists, and physical therapists, to coordinate patient care.
- Educate patients and their families on speech therapy techniques, exercises, and strategies for home practice.
- Maintain accurate and up-to-date patient records and documentation.
- Ensure compliance with all relevant laws, regulations, and ethical standards governing speech therapy practice.
Qualifications:
- Minimum 1 year experience in an Adult Home Health environment
- Master's degree in Speech-Language Pathology from an accredited program.
- Valid state licensure as a Speech-Language Pathologist.
- Previous experience with Kinnser / WellSky EHR system
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
- CPR certification (BLS for Healthcare Providers).
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Reliable transportation and willingness to travel to patients' homes.
