Registered Nurse Case Manager - Field Supervisor
Location: Palmdale and Lancaster

We primarily offer in-home nursing care to pediatric and adult patients with, feeding tubes, tracheostomies, and/or ventilators.
RN/REGISTERED NURSE RESPONSIBILITIES:
· Submit OASIS forms for all patients on time
· Supervise and manage the delivery of nursing care to clients
· Reviews patient’s clinical need, updating diagnosis, medications, and procedures
· Conduct start of care visits at patients’ home
· Coordinate clinical notes and plans of care
· Review and update plans of care during home visits
· Train new field team members
· Attend periodic in office meetings
· Counseling and Supervision of Clinical Staff
· Educate patients and caregivers on disease process, medications, plan of care and treatment plans
RN/REGISTERED NURSE QUALIFICATIONS:
· Registered Nurse (Bachelor or Associate Degree) with at least 2yrs of clinical experience
· Home health experience preferred
· Proactive approach with a problem-solving mentality
· Strong management skills with the ability to supervise field staff
· Valid nursing license, CPR license, and driver’s license (travel required)
· Current PPD or chest x-ray required.
· Fluent in English, Bilingual Spanish preferred
· Highly organized and ability to work independently
· Computer literate with the ability to learn new applications
Job Type: Full-time
Pay: $80,000.00 - $85,000.00 per year
