  • Three (3) current letters of reference from healthcare providers familiar with the contractor's clinical practice, including one from the Chief of Staff / Medical Director or Immediate Supervisor.
  • Must have worked within the last 6 months

    • Required Health Documentation :

  • MMR, Hep B, Varicella, Tetanus (Td), PPD, Flu

    • Required Certifications :

  • BLS, ACLS, PALS, ATLS
