Responsibilities
- Conducts comprehensive patient assessments and develops care plans.
- Identifies and resolves discharge obstacles promptly.
- Acts as a resource for treatment decisions and end-of-life issues.
- Facilitates discharge plans and transfers to other facilities.
- Communicates effectively with patients, families, and team members.
- Participates in quality improvement activities.
- Develop comprehensive multidisciplinary plans of care, effectively utilizing tools and resources
- License: Any state current and valid license
- Certifications: BLS
- Experience: 2 years’ directly related experience or Experience in acute care management, discharge planning, performance improvement (preferred)
