Job Title: LPN - Long-Term Care/Nursing Home
Location: Charleston, SC and associated CBOCs
Shift Schedule: Between the hours of 7:00AM and 5:30PM (Monday through Friday, 12-hour shifts, including alternating weekends.)
Duration: 1 Year or Full time (options for 26 weeks contract)
Start Date: March 2025
Qualification
Education: Graduate of a state-approved school of practical/vocational nursing.
License: LPN
Certifications: BLS/CPR and Covid 19
Experience: Min 2 years of recent experience in a long-term care setting (within the last 6 months).

Responsibilities
- Deliver nursing care in accordance with the Licensed Practical Nurse Act and VA guidelines.
- Administer and document medications (oral, topical, subcutaneous, intramuscular, and intravenous) accurately.
- Assist patients with daily living activities, monitor vital signs, and report changes in condition to RNs or physicians.
- Provide patient education on health conditions, treatments, and coping strategies.
- Maintain accurate patient records in VistA/CPRS systems.
- Support infection control measures and adhere to safety protocols.
- Provide care to COVID-19 positive patients or PUIs as required. Refusal to do so may result in removal from assignment.
