**Job Title:** LPN / LVN Long Term Care
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team as a travel healthcare professional in Amery, Wisconsin.

Responsibilities:

- Conduct thorough assessments of patients' physical and emotional needs, developing individualized care plans to address these needs
- Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes
- Maintain accurate and detailed records of patient interactions, progress notes, and treatment plans
Requirements:
- Current licensure as an LPN or LVN in the state of employment
- Experience working in long-term care settings preferred
- Excellent clinical skills and ability to work independently
- Strong communication and interpersonal skills
- Ability to work flexible schedules, including evenings and weekends
