Key Responsibilities
- Deliver high-quality physical therapy services to patients in a SNF, ALF, or ILF setting, promoting optimal outcomes and independence.
- Collaborate with the multidisciplinary team to develop and implement individualized treatment plans that address the unique needs of each patient.
- Ensure seamless communication with patients, families, and healthcare staff, maintaining open lines of dialogue and fostering a culture of teamwork.
- Document patient interactions, including assessment results, treatment plans, and progress notes, ensuring accurate and timely recording.
Requirements and Qualifications
- Active licensure as a Physical Therapist in the state of Massachusetts.
- At least 1 year of experience as a Physical Therapist in a SNF, ALF, or ILF setting.
- Exceptional communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare staff.
- Capacity to lift up to 50 pounds, stand for extended periods, and engage in frequent bending and stooping.
Benefits
