Main Responsibilities
- Deliver top-notch physical therapy services to patients in a SNF, ALF, or ILF setting, prioritizing patient-centered care.
- Foster close collaboration with healthcare professionals to design and refine treatment plans tailored to individual patient requirements.
- Ensure clear communication among patients, families, and healthcare staff, guaranteeing cohesive care delivery.
- Accurately document patient interactions, including assessments, treatment plans, and progress notes.
Specifications and Needs
- State licensure as a Physical Therapist in good standing.
- At least one year of experience as a Physical Therapist in a SNF, ALF, or ILF setting.
- Exceptional interpersonal and communication skills, with the capability to establish trusting relationships with patients, families, and healthcare staff.
- Ability to lift up to 50 pounds, sustain standing positions, and frequently bend and stoop.
