TITLE: Case Manager II
REPORTS TO: Program Director
APPOINTED BY: CEO
BASIC FUNCTION: Provide intensive case management services for homeless/ at risk of homeless families
- Provide therapeutic case management, community support and counseling services in the areas of daily living needs.
- Support families surrounding the obtaining and maintaining mainstream benefits.
- Provide viable referrals and linkage relating to needs identified on the family/individual service plan.
- Provide support and resources to adolesents and youth in transition.
- Provide resources for employment, school enrollment and increasing income.
- Collect program-based intake paperwork requirements.
- Complete paperwork and enter data into requirement data-based systems (HMIS, Client Serve, STARS etc.) in a timely manner.
- Participate in programmatic meeting and clinical rounds.
- Be familiar with Medicaid rules and regulations.
- Complete Mental Health Assessments/Treatment plans and enter into the Electronic Records System.
- Administer Diagnostic Assessment Tools
REQUIREMENTS:
- At least a BA or BS degree, preferably in social work, human services or related field
- At least three-five years of case management experience, preferably with homeless families and/or survivors of domestic violence or human trafficking
- Deep understanding and commitment to the issues facing families surviving violence, abuse and trauma.
- Ability to engage high-risk and vulnerable families.
- Demonstrate sound organizational, solid verbal, written and computer skills
