The Prince George's County Department of Social Services (DSS) is currently seeking highly qualified applicants to fill a Transitional Housing Rapid Rehousing Program (THRRP) Case Manager (Community Developer 1G) position Grade G18 in the Community Services Division.
About the Position
The Case Manager is responsible for assisting homeless families in the Transitional Rapid Rehousing (THRRP) and HELP Program. This position serves as a member of the Department’s Community Services Division and is supervised/evaluated by Program Manager.

We do this by providing intervention services that strengthen families, protect children and vulnerable adults, encourage self-sufficiency and promote personal responsibility. We assist our customers through such programs as Temporary Cash Assistance, Food Supplement Program, Medical Assistance, Emergency Assistance, and Foster Care and Adoption Service.
- Completes client applications on day of intake.
- Reads and explains resident admissions agreement on day of intake.
- Completes and places new participant’s documents in case record: driver’s license, social security, pay stubs etc. on day of intake.
- Assists new participant in filling-out program forms and place in case record on day of intake.
- Explains and provides new participants with program rules and regulations on day of intake.
- Takes urine samples from new participants for drug testing on day of intake.
- Develops service plans, establishes goals for new participant and document in case record at initial case management session.
- Sets realistic timelines for established goals and documents in case record at the initial case management session.
- Assess and refer new participant to job development and/or training program at initial case management session.
- Provides program participant with updated housing lists.
- Assists program participants in applying for services (SSI, Food Stamps, Medicaid, etc.) as needed.
- Provides ongoing monitoring to ensure families receive the services they need and are eligible for.
- Provides transportation to families for housing searches as needed.
- HMIS - Inputs data on program participants and updates the pertinent information and service records as needed.
- Meets with program participants weekly to monitor progress and establish new goals with timelines.
- Takes urine samples from program participants for drug testing bi-weekly or as needed.
- Collects and place program participants proof of income and proof of savings in case record monthly.
- Documents case management session in case record weekly.
- Monitors (weekly) participants attendance to program meetings.
- Refers and links program participants to appropriate resources weekly or as needed: housing, employment, training, clothing food, etc.
- Contacts/Identifies and document established and potential resources for program participants weekly.
- Provides follow-up on referrals and contacts the agency to ensure that services have been provided.
- Receives and distributes donations weekly or as needed.
- Assists customers in assessing the needs for their family on an ongoing basis.
- Contacts local businesses and organizations to identify and secure the appropriate resources for program participants as needed.
- Attends meetings with program participants and serves as an advocate and mediator at the customer’s request.
- Willingly and cooperatively performs tasks and duties in which may not be specifically listed in the position description but are within the general occupational category and responsibility level typically associated with the employee’s class of work.
- Bachelor's Degree with major course work in business or public administration, social service, sociology, psychology, or other fields
- Responsible work experience which includes coordination of group activities, preferably including work with disadvantaged individuals

- Ability to excel in a team environment.
- Ability to lead, inspire, motivate subordinate staff or volunteers, and delegate responsibility to them as appropriate.
- Experience leading or working in a large and fast-paced urban/suburban Human Services Agency.

Salary : $43,014 - $52,000
