What are the responsibilities and job description for the Patient Educator Nurse position at Princeton Community Hospital Association, Inc.?
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
2. Obtain certification in Basic Life Support within 30 days of hire date.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. They are not intended to be constructed as an all-inclusive list of all responsibilities and duties. Other duties may be assigned.
1. Identifies patient education needs through unit rounds, discharge planning meetings and conferences with other healthcare professionals. Ensures clinical records reflect patient's educational needs. Documents teaching and patient/family/support person's understanding.
2. Assists with community education programs and screenings such as community health fairs. Responsible for the organization and maintenance of the Diabetic support group.
3. Responds timley to triggers requesting patient/family education and/or wound care. Provides new ostomy teaching and complex ostomy support.
4. Provides Diabetic education regarding overall disease management, diet, exercise, operation of glucometers and insulin injections. Provides education on Wound Care/Wound Vac.
5. Works with Nurse Educator and provides staff education when necessary.
6. Provides education to patients/family/support person for patients with Aspira drains.
7. Provides patient education as needed on topics such as heart failure, pneumonia, sepsis, new onset atrial fibrillation and MI. Provides patient education as needed for skills such as Lovenox injections, tube feedings, trach care.
8. Completes Annual Completion of Diabetes, Wound Care or Ostomy education.
