Title: Activities/Transportation Aide
Department: Activities
Reports To: Activities Director
General Overview:
Incumbent responsible to assist Activity Director in discharging duties and responsibilities. To act as driver and escort for resident appointments (hospital, doctor's office, etc.)
Specific Duties and Responsibilities for Activities:
Assist with the development of the recreation and activities program.
Assist with the organizing and directing of the execution of the recreation and activities plan.
Assist with maintaining a current record of community services, resources, programs and facility materials accessible to staff, residents, and resident s relatives and friends.
Assist with ascertaining from the resident's care plan any physician s order limiting participation in the activities program.
Assist with documenting resident's participation in activities and reasons for non-participation in the activities program.
Assist with documenting residents' participation in activities and reasons for non-participation in otherwise appropriate activities.
Assist with the coordination of the volunteer program.
Specific Duties and Responsibilities for Transportation:
Assist/escort residents on any necessary trips out of the facility, to include but not limited to: Banking, physician appointments, dental appointments, shopping, etc.
Assist, perform any specialized purchasing that PHCC needs-trips to bank, post office, emergency trips for pharmaceutical, food purchases, etc.
Responsibilities:
Providing safe, pleasant, comfortable transportation to our residents while they are conducting their affairs.
Qualifications:
High School diploma or equivalent.
Valid Driver's License.
Certified Nurse Aide license is preferable but not a requirement.
Able to work flexible schedule including evenings and weekends.
